Please note that some of the issues listed below can be resolved with the proper protocol. Therefore, if it is discovered that at any time you were untruthful in answering the questions below, you will be prohibited from booking with Fresh Faced Permanent Makeup, as it may potentially cause harm to you or the Artist.

  • Do you have diabetes?

  • Are you currently pregnant or nursing?

  • Do you have glaucoma?

  • Do you have any blood disorder that may affect clotting?

  • Do you have any history of keloid/hypertrophic scarring ?

  • Do you have any heart conditions?

  • Do you have an artificial heart valve or artificial joints?

  • Do you have any Permanent Makeup on the area you want us to perform a new procedure?

  • Are you allergic to any of the following; metals, hair dye, tattoo pigment, food coloring, anesthetic?

  • Do you have high blood pressure?

  • Do you take Accutane or topical steroids?

  • Do you take anticoagulant (commonly referred to as blood thinners) such as Coumadin/Warfarin, Heparin, Eliquis, Lovenox, Xarelto or Plavix? 

  •  Have you undergone Chemotherapy within the last 12 months?

  • Do you have any history of HIV/AIDS?

  • Is there sunburn, peeling skin, rash, or lesion on area you wish you have micropigmentation performed?​
